Return your completed and signed application to 69 Fairlands Road, Sale M33 4AU by 30 April, together with a stamped addressed envelope.  You will need to affix a “Large” stamp when you send us your application form but a normal stamp will be sufficient for the SAE, which will be used to notify you of your allocations.  If you do not have access to a printer please visit a library or ask a friend to download the forms on your behalf, as we do not have the capacity to post application forms out.  Details of the proposed activities will be posted on the individual scheme pages as soon as they have been confirmed – please check it on a weekly basis and let us know as soon as possible if your youngster won’t enjoy the activity that is scheduled for a day when they are due to attend, we will attempt to offer an alternative day.

Please make sure that your Social Worker or Family Liaison Officer knows just how valuable our service is to your family and ask them to fund your youngster’s place at the fully costed rate of £60 per day.  All three Boroughs have agreed to fund future places at this rate but they are still paying the voluntary contribution rate for some existing users and do not provide any funding for others. If your place is unfunded the cost will be £20 per day – the number of unfunded places that we can offer may be restricted due to funding constraints.

We will once again be requesting payment in advance via bank transfer – refunds will only be made in extenuating circumstances and places will be withdrawn if payment is not received by the pre-advised deadline.  Our bank details will be included in your allocation letter.

If you have any questions then please contact us.

Chairman
